What Is Occupational Therapy?

Kawa is the Japanese word for ‘river’. The Kawa Model employs the metaphor of a river to effectively symbolize an individual’s life journey, highlighting how life flows in various directions. Just as a river can meander smoothly through tranquil landscapes, it can also encounter obstacles such as rocks and fallen trees, representing the challenges we face in our own lives. By utilizing the Kawa Model, occupational therapists and clients can gain a deeper understanding of the factors that influence that natural flow and collaborate to implement strategies for improvement.

Using the Kawa Model as an Assessment

The Kawa Model is a qualitative, conversation-based assessment. There is no fixed order or script. The therapist follows the client’s narrative, allowing priorities and meanings to emerge naturally. The client’s perspective guides the occupational therapy process*.

Creating Space for Change

Intervention focuses on creating or widening ‘spaces’ in the river so life can flow more freely. This may involve reducing the impact of challenges (rocks), adapting environments (riverbanks), or using existing or new strengths (driftwood) to improve flow*.
